RISE, a leading organization dedicated to promoting health equity and addressing social determinants of health, has unveiled the finalists for its prestigious 2024 Health Care Hero Award. The award honors individuals who have made exceptional contributions to improving the lives of underserved populations through healthcare and social services interventions, exemplifying RISE’s mission to promote health equity among all patients.
From a pool of 59 nominations, five outstanding finalists have been selected for their remarkable efforts in addressing the complex challenges faced by underserved communities. The finalists include Cristina Conlon, a care navigator at ReferWell; Tiffany Davis, the executive director of quality improvement and health equity at Blue Cross Blue Shield of Illinois – HCSC; Dr. Kimberlee Wyche Etheridge, a primary care provider, assistant professor at Meharry Medical College, and senior VP of health equity and diversity initiatives at ASTHO; Sirene Garcia, the CIO of Finger Lakes Community Health; and Megan Shreve, the CEO of South Central Community Action Programs, Inc.
The winner of the 2024 Health Care Hero Award will be announced at The RISE Summit on Social Determinants of Health in New Orleans on April 16, the second day of the three-day conference. The award recognizes individuals who have made significant strides in addressing social determinants of health, working for organizations such as health plans, healthcare providers, social service providers, non-profits, government agencies, foundations, pharmaceutical companies, or vendors/suppliers.
RISE is now accepting nominations for the 2025 Health Care Hero Award, with submissions open until December 31, 2024. The award aims to celebrate individuals who have contributed to significant process improvements in addressing the care of underserved populations or dedicated more than ten years to exemplary work in this industry.
